Pre-const. Mtg. Reqd call dps inspector malvin nicholson @ 301-370-3685 place, splice and test aerial/buried fiber optic cable facilities (conduit/ducts/handhold boxes/pedestals,etc). All above & below grade facilities shall be placed in public utility easements or adjacent to the right-of-way line unless approved otherwise by the mcdps right-of-way (row) inspector. Work shall comply with the montgomery county specifications for utility const. Permit, july, 2003. All lane closures/traffic controls shall comply w/approved traffic control plans, guidelines for temporary traffic control requirements & directions of the dps row inspector. Complete repair/restoration of the row shall be made in-kind of any & all damages to improvements in the public row caused by construction operations. On old gate road (#70 - existing school) replace pole, transfer overhead wire, install 4 amp underground service, dig and backfill trench, build conduit, install cable and associated equipment in accordance with pepco plan no. 63134152. Complete repair (restoration of right of way) shall be made of any and all damages done to the existing improvements in the public right of way caused by construction operations on this site. Pre-construction meeting requiredcontact mcdps inspector bob bucklin at 301-370-3683 before and after installation.

On old gate rd (7000) dig and backfill trench, build conduit, repalce and install cable and associated equipment to provide services in accordance with pepco plan no. 3334903. Work shall comply with the current montgomery county specifications for utility construction permit and the direction of the dps inspector. Complete restoration shall be made of any and all damages done to the existing improvements in the public right of way caused by construction operations on this site. A mill and overlay shall be required should the patch not meet the specifications as set forth by the current county specifications for utility construction permit and as directed by the dps row inspector. All lane closures/traffic controls shall comply with the mcdot approved traffic control plans, work zone temporary traffic control standards and directions of the dps row inspector.

Construct intersection type driveway as shown on the attached approved plan. Driveway will consist of montgomery county standard 100. 01 curb and gutter. Construct concrete sidewalk (standard mc-110. 01), sidewalk ramps (standard mc 112. 01) at all sidewalk crossings, install 15-inch rcp storm drain and water sewer connections as shown on approved plan. Pavement milling & overlay is required for street cuts under this permit as shown on the approved plan. Prior to release of this permit, complete repair & restoration of the right of way shall be made, "in-kind", of any and all damages done to the existing improvements in the public right-of-way caused by construction operations on this site. Disturbed areas shall be fine graded & sodded. Roadways shall be kept free of mud & debris. Materials & equipment shall not be stored in right-of-way.

Intersection type driveways: construct intersection type driveways with ada ramps, sidewalk and stormdrain connections; remove and plant trees; install traffic sign as described on the attached approved plan. Complete repair (restoration of right of way) shall be made of any and all damages done to the existing improvements in the public right of way caused by construction operations on this site. Work shall be completed as directed by the dps inspector.
