Construct an intersection type driveway consisting of mont. Co. Standard 10-a curb and gutter. All unsuitable sub-grade material shall be removed and replaced with approved material prior to placement of concrete curb and gutter or base paving. Provide sidewalk ramps standard mc112. 01 or mc113. 01 as directed at all sidewalk crossings. All vehicular access to this site shall be gained via this entrance. This permit will be revoked immediately should any vehicle or construction equipment cross the curb and/or sodded shoulder and ditch within the right of way(row). Install a 6" water house connection. Pavement repair shall be done in accordance with the requirements of "specification for utility construction" and as directed by mcdps inspector.

Installing 65 of 6" plastic-hp main and 2" plastic hp service to serve 2011 e. Jefferson street. Plan no. Nbr 45106. No pavement cut is allowed. Amendment (1/4/2001): pavement on a portion of the installation parallel to the roadway approximately 65 is allowed. Provide a 1 1/2" single lane mill and overlay over the propsed pavement cut. All work must be coordinated with mcdps inspector.
