Notice to proceed all work to be completed as per plans in chap files. Install new replacement roof to match as per code repair leaks as needednon-structural alterations/interior work - replace 3 interior doors same size and location. Exterior work - new roofing. As per code.

Amend permit#com2016-11903 issued on 5-9-16 to add the following additional work; 7 new ceiling fans 7 switches 1-220v for a/c unit outlets. Total of 25 fixtures/devices. Work done to code. Install a new 1/2" copper re-circulating hot water return 1st floor powder room- disconnect and reconnect toilet. 2nd floor bathroom- disconnect and reconnect toilet. 3rd floor bathroom- disconnect and permanently cap. Install a dual zone ductless unit ( exterior) for the it room and printer/fax room as per code. Per chap: atp: install new mitsubishi a/c unit on the 2nd floor of the side elevation above the fire escape. All work must match drawings on file with chap.
