Nstall three 8x8" plates per structural engineer specifications, install additional 8"x8" plate below 2nd floor window located at 2nd floor joist, remove bowed brick section per stuctural engineer specifications and reinstall. Permit application in response to unsafe violations. To comply violation, completed work is to be inspected and approved as an appropriate repair to each violation. Licensed contractor to assume responsibility of work. No other work this permit. Separate permits required for renovation. Contact streets department for dumpster and or footway closure permits. The listed general contractor, patrick kane masonry, has no employees as per notarized letter. All sub-contractors are to be licensed in the city of philadelphia, and listed on permit. A sub-contractors list is to be given to the inspector before any work starts.
