Rebuild revision submitted 7/20/11 to change wording on rooms and add wall;ram

On bentley la (house #10711) replace overhead wires, make a 1x1 rd cut to check for other utilities, dig and backfill trench, build conduit, install cable and associated equipment to provide 4 amp underground service in accordance with pepco plan no. 3355793. All above & below grade facilities shall be placed in publ. Util. Easements or adjacent to the right-of-way line unless approved otherwise by the mcdps row inspector. Work shall comply with the current mc specifications for utility const. Permit. All lane closures/traffic controls shall comply with the mcdot approved traffic control plans, work zone temp. Traffic control stds and directions of the dps row inspector. Complete repair (restoration of right of way) shall be made of any and all damages done to the existing improvements in the public right of way caused by construction operations on this site. Pre-construction meeting requiredcontact mcdps inspector before and after installation.

Need to replace revised plan (engineer signed) dw permit: construct mc-301. 01 type driveway in accordance with montgomery county standards and specifications and the direction of the dps inspector. Complete repair (restoration of right of way) shall be made of any and all damages done to the existing improvements in the public right of way caused by construction operations on this site. The applicant may be required to obtain a separate roadside tree permit from the maryland department of natural resources forest service. Contact the montgomery county department of transportation, division of highway services for county arborist to inspect the site for tree removal or appropriate species for tree planting within county rights of way prior to driveway construction.
