Make safe permit - front wall bulge. Installation of 3 star bolts per engineer letter. Temporary bracing installed to stablize front wall. For (brief description of work as per engineers report if applicable) to resolve case 532719. Additional engineering report inspection needed after installation of star bolting to determine additional repair or designed permanent repair to be implemented. Abutting sidewalk must be closed with fencing a minimum of 6 in height. Separate streets department permit required for sidewalk closure. A separate permit is required for any additional alterations that are not specifically addressed on case 532719. Contact area inspector prior to start of work.
